To register on-line, complete the Subscriber Registration Form below.
In order to insure that our materials will be available for those
who need them, certification of your visual impairment, physical
disability, or reading disability is required. This will entail your
sending us a letter from an ophthalmologist, health or social services
professional, teacher or librarian. Your registration will be
completed on receipt of certification. Mail or fax this

Please note that if you're already registered with an agency for the blind and visually impaired in your state, all we require is the name of the agency and your certification number. Fill in this information on the appropriate line of the Subscriber Form.
